PRP and Shockwave Therapy: A Synergistic Approach to Healing and Performance

When it comes to performance health and longevity, regenerative therapies are advancing faster than ever. Two modalities now available at Cenegenics Beverly Hills are: platelet-rich plasma (PRP) therapy and extracorporeal shock wave therapy (ESWT, also known simply as “shockwave”). These treatments combined create a powerful new pairing. What Is PRP? 

Platelet-rich plasma (PRP) therapy uses a patient’s own blood to concentrate platelets and growth factors that naturally promote healing. After a simple blood draw and centrifugation process, PRP is injected into targeted tissues, delivering a potent cocktail of signaling molecules that stimulate repair, angiogenesis (the formation of new blood vessels), and tissue regeneration. Because PRP comes from the patient’s own body, it carries no risk of immune rejection and is an especially attractive source of growth factors for vascular healing.  

 

What Is Shockwave (ESWT)? 

Extracorporeal shock wave therapy (ESWT) uses high-energy acoustic pulses generated outside the body to stimulate localized healing responses. These rapid pressure waves induce controlled microtrauma in the target tissue — a signal that increases blood flow, stimulates the release of growth factors, and activates cellular repair pathways.  

There are two main types of shockwave used clinically: focused ESWT, which delivers energy to precise depths within tissue, and radial pressure wave therapy, which disperses energy more broadly near the surface. Focused approaches are often chosen for vascular repair and deeper musculoskeletal injuries, while radial systems can be beneficial for broader, surface-level conditions like certain tendinopathies. 

Initially developed for breaking up kidney stones, ESWT has since become a proven treatment for musculoskeletal conditions like tendinopathies, calcific shoulder tendinitis, plantar fasciitis, and non-union fractures. Studies also show promise in vascular applications, including erectile dysfunction (ED) and Peyronie’s disease.  

 

Why Combine PRP and Shockwave? 

Each therapy alone is effective, but together PRP and ESWT create a synergistic effect that neither can achieve on its own. 

  • Shockwave directs PRP’s healing power. ESWT induces microtrauma that not only stimulates natural repair, but also creates a “pathway” guiding the concentrated PRP growth factors directly to damaged tissue. 
  • Accelerated vascular repair. This combination promotes angiogenesis, breaks down microplaque, and restores healthy blood flow — all critical for both vascular and musculoskeletal recovery. 
  • Tissue remodeling. The pairing enhances collagen formation and tissue regeneration, improving structural integrity while relieving pain. 

In short, shockwave “primes the soil” and PRP “plants the seed” — together fostering deeper and faster healing. 

 

Applications in Erectile Dysfunction and Peyronie’s Disease 

For men with vascular-based ED, diminished blood flow and microplaque buildup in penile tissue can make conventional treatments ineffective. PRP injections appear promising in vasculogenic ED, especially when paired with ESWT, showing improved erectile function over time. PRP and ESWT together address the root issue by: 

  • Promoting angiogenesis and improved penile blood flow 
  • Breaking down fibrous plaque associated with Peyronie’s disease 
  • Supporting regeneration of vascular and nerve tissue 

Recent randomized trials confirm that combining PRP with low-intensity shockwave therapy results in significantly greater improvements in erectile function compared to shockwave alone.  

This makes the combination especially valuable for patients with severe ED or Peyronie’s, where restoration requires more than symptomatic relief. 

 

Applications in Tendinopathies and Orthopedic Conditions 

Tendons heal slowly, often leaving athletes and active adults frustrated by chronic pain or recurring injury. ESWT has long been used to break down calcifications, stimulate circulation, and reduce pain in tendinopathies. Clinical reviews demonstrate that ESWT produces measurable improvement in pain and function within 3–12 weeks, with benefits that persist for up to two years. Adding PRP amplifies the healing cascade by delivering growth factors that actively remodel tendon tissue. 

Together, the therapies can accelerate recovery in conditions such as: 

  • Tennis elbow and golfer’s elbow 
  • Achilles and patellar tendonitis 
  • Shoulder and knee tendon injuries 
  • Plantar fasciitis 

In fact, case reports of runners with plantar fasciitis have shown that combined PRP + ESWT delivers faster pain relief and better functional recovery than either therapy alone.  

This approach doesn’t just quiet symptoms.  It enhances repair at the biological level, reducing recurrence and supporting long-term performance.
