Cancellation Policy:
- You may cancel at any time by providing your request to cancel in writing to [email protected] or 6830 Spencer Street Suite 102 Las Vegas, NV 89119
- If you paid the annual fee, you can get refunded the prorated portion of the program fee, less the list price of the services
- If you paid the monthly fee, you won’t be charged additional program fees but you will be charged the list price for any services you have already received
Below is the cancellation policy in detail:
By signing up for the Program, you agree to its automatic renewal feature until you cancel. The Program will automatically renew at or around each anniversary of the Effective Date. Each renewal year shall be for one (1) year. For as long as your Program has not been canceled, prior to the start of each Program year, Cenegenics will send you notice by email that your Program is about to be renewed, along with a renewal invoice. You will have 30 days after the date of the invoice to opt-out and decline to purchase the Program for the upcoming and subsequent years. If you do not opt-out during the 30-day period, your Program will be automatically renewed for the next year and you will be responsible for the full Program Fee for the renewed year. The Program Fee for the initial year may be increased by up to 25% per renewal year. The amount of the Program Fee will be stated in your renewal notice. If you do not timely opt out of the renewal, your credit or debit card on file will be charged on or around the anniversary of the Effective Date either (1) the full amount of the Program Fee less the approximate 10% discount; or (2) the first of 12 monthly installments of the Program Fee, depending on the option you choose on your Payment Authorization. Any remaining Program Fee installments will be charged to your credit or debit card on or around the same day of each month. You may cancel the Program at any time effective 30 days following notice to Cenegenics. Purchase of the Program for the year is final. Cancellation of the Program will terminate only the automatic renewal feature of your account. No refunds will be made and scheduled installment payments will continue to be charged until the end of the purchased Program Year except:
If the Program Fee was paid in full, the prorated portion of the Program Fee for the canceled months, less the list price of services (e.g., lab panels) rendered ahead of schedule,* will be refundable.
If the Program Fee is being paid in monthly installments, scheduled installment payments will terminate for the canceled months upon payment in full at the list price* of any services (e.g., quarterly diagnostics or lab panels) that have already been provided.
* You will be provided a copy of this schedule and list prices.
Cenegenics reserves the right to cancel the Program at its sole discretion, including for failure to pay installments of the Program Fee. Cenegenics further reserves the right not to provide the Therapy if the Program Fee is not paid in full or in accordance with the monthly payment schedule. If the event your credit or debit card number changes, the authorizations you have made hereunder will remain in effect and shall apply to the new account number. Cenegenics reserves the right to modify these Terms and Conditions. In the event of a material change in these Terms and Conditions, Cenegenics will provide you with notice of the material change and information on how to cancel the Program.
